Description of work at the actual clinic / health centre
The geriatrics department is part of the special medicine business area. Specialized medicine is tasked with offering emergency and planned specialist care for the population of region Gävleborg. The business area includes: infection, skin, neurology, rheumatology, geriatrics, medical rehabilitation as well as asylum and migrant health.
We are a care department withcare places, of which 8 are geriatric rehab and 4 are emergency geriatric places. In the department, doctors, nurses, sub-nurses, occupational therapists and physiotherapists are working. When the need arises, we also have access to a speech therapist, dietitian and curator.
In the department, a specialist and usually 2 sub-doctors work. The specialist has the main responsibility for all patients while the sub-doctors divide the patients between themselves. Working hours are weekdays from 8 am to 4:30 pm. We have a pharmacist in the department 1 day a week who assists with drug reviews and participate in the rounds if necessary.
We have a situation-oriented way of working, which means a total care for the elderly multi- sick. Treatment is based on the individual's needs. Training takes place in everyday situations and aims to make the patient as independent as possible.
The department strives to work closely with patients, relatives and other care providers.
Patients can be admitted via the emergency room, from other care units or directly from the home / accommodation via family doctor.
We have a progressive approach to geriatric care and work with assessment and management of our patients according to CGA.
A memory reception is also attached to our clinic, which is located in another part of the hospital. We also have a memory reception in Hudiksvall. We have no doctors on duty in the evenings and nights, but during the weekends the specialists take turns rounding the department. At other times, the nurse may, if necessary, seek medical advice.
At the Memory Reception, different types of dementia are investigated and diagnosed on referral from primary care and outpatient care. At the reception there are doctors, nurses, occupational therapists and, if necessary, curator and neuropsychologist.

Plans for the future (if the activity will be developed in any particular direction, organizational changes etc.)
Sweden's regions and county councils together are facing a structural change in Swedish healthcare. The goal is a good and close care: a coordinated and person-centered care close to the patient that is given on the right level of care and at the same time is financially sustainable over time. In order to give ourselves the opportunity to continue to offer good care, we must develop care based on the real needs of the residents in the coming years. A good and close care offers the best solutions based on local conditions and the wise use of our common resources.
The care is coordinated and centred around the patient, we benefit from modern digital technology and focus more on preventative efforts. The most important ingredients are high availability and a good, personal treatment. We listen to each patient's unique needs and conditions, and make the patient participate in their care.
